This is a Sturm Geschutz (Storm Gun) Mark III, featured at the WWII re-enactment in Rockford, Illinois, September 2012
This is not a tank, it is a self-propelled artillery piece built around the hull of a Panzer. Very few still exist today.
19/10/2017: After five years of going back and forth on a positive identification, I'm finally tracked down the group's website. It is a Mark III after all, but built from spare parts, resulting in a non-standard appearance.
